Calderdale Over 40s Health Survey

This survey has been created to help CalderGold understand the health, wellbeing and community needs of people aged over 40 across Calderdale.

We want to learn which services people already use, what support may be missing, what prevents people from looking after their health and how much interest there is in a dedicated CalderGold Health & Wellbeing Centre.

Your answers will help us plan services, improve the local information available through the CalderGold website, demonstrate community demand and support future discussions with funders, partners and health professionals.
